Boston, MA: Geiger Gibson Community Health Center, Harbor Community Health Center Hyannis, and Neponset Health Center have been awarded Level 3 Recognition by the National Committee for Quality Assurance (NCQA) Physician Practice Connections® (PPC) -Patient-Centered Medical Home ™ (PCMH).
Harbor Health Community Health Center divisions were among the first Community Health Centers in the Nation to apply and receive
the highest level of recognition for a PPC- PCMH in 2010. The current award is a result of a rigorous application to receive PPC-PCMH recertification.
The Patient Centered Medical Home standards emphasize the use of systematic, patient centered, coordinated care that supports access, communication and patient involvement. The 16 physicians and 15 nurse practitioners working at the aforementioned health centers are among a handful of providers in the State to receive this recognition. The three health conditions that were chosen as a part of the evaluation by NCQA were diabetes, asthma and pediatric care.

During the past several years, our Community Health Centers became the epitome of the Patient Centered Medical Home facilitating care for our patients through ongoing patient participation, progressive care coordination working side by side with local hospitals and specialists, the development and use of high risk patient registries, electronic medical records and a robust bilingual patient portal. Harbor Health Services, Inc.’s first Center was the nation’s first Community Health Center in Dorchester, Massachusetts, now known as the Geiger Gibson Community Health Center. That Center opened in 1965.
